Note: Our district payroll is run monthly with payday falling on the last working day of each month.
Notice of Electronic Delivery of Form 1095-C
Quilcene School District provides Form 1095-C to eligible employees and covered individuals electronically.
What This Means
Instead of receiving a paper copy by mail, you may access your Form 1095-C securely online.
How to Access Your Form
Your Form 1095-C will be available in your Skyward Employee Access
(username and password are needed to login)
Requesting a Paper Copy
You may request a paper copy of your Form 1095-C at any time at no cost.
Withdrawing Consent
You may withdraw your consent to electronic delivery at any time by contacting the HR/Payroll Office Once consent is withdrawn, future Forms 1095-C will be provided in paper format.
Updating Your Contact Information
It is your responsibility to keep your email and mailing address current. Please update your information by contacting HR/Payroll Office.
